Taking care of yourself

Our Health Partnerships division run the Nutrition and Dietetics service in Mid Nottinghamshire. This service provides therapeutic dietetic advice to people with a range of dietary issues including diabetes, malnutrition, enteral feeding, coronary heart disease, gastroenterology, food allergies, obesity and elderly care.

Patients requested more involvement in their medication and in the planning of their care. They commented on wanting more independence, and the capacity and understanding to take care of themselves more, rather than relying on the expert knowledge of staff.

As a response, all staff in the team have been trained in a range of self-care management techniques. This means they are able to pass on this knowledge to patients, enabling them to understand their medication and their condition better and take better care of themselves between appointments with clinicians or pharmacists.

This is a good example of the way well informed patients, with the help of knowledgeable and supportive staff, can take control of their own lives more, and remain in the community, at home, and living well.
