New Adult Community Care Service set up in response to feedback

Adult community teams (e.g. community nurses, matrons, intermediate care, specialist nurses) in Nottingham North and East (Gedling area) have been working with the Clinical Commissioning Group (CCG), patients, carers and community organisations to find out what people want from community health services. As a result, the new Adult Community Care Service has been set up to provide patient centred high quality co-ordinated care at home and in community clinics. This means that our staff work more closely with colleagues, patients and their families to plan their care, coordinating it to ensure that people are seen by the right person at the right time for the best possible care and treatment.We have also set up a new Single Point of Contact (or Access - SPA) for patients or carers to call with any worries or concerns. People can call SPA between 8am and 6.30pm Monday to Friday on 0300 083 0100. If anyone has immediate health concerns there is also an experienced clinician available to talk to, and staff available to come out quickly if they are in crisis.
